Does the MAPD Plan require referrals?

No, you do not need referrals with the Humana MAPD plan.

Does the Humana MAPD Plan have a network?

No, you can go to any provider, hospital, or facility that accepts Medicare and is willing to bill the Humana
Group Medicare Advantage (PPO) Medicare Network. Your benefits are the same in- and out-of-network.

Can I continue to see my current providers?

Yes, you can see any provider that accepts Medicare and is willing to bill the Humana Group Medicare
Advantage (PPO) Medicare Network. Please contact your IAFF HWT Retiree Advocates at (360) 205-
1415 or Toll Free (844) 818-1088 (TTY 711) with any questions or concerns. Your providers do not have
to be contracted with Humana to accept this plan.

What if my provider says they do not accept this plan?

If your provider accepts Medicare, they will be paid the same by the plan regardless of if they are
considered in- or out-of-network. Please call RetireeFirst at (360) 205-1415 or Toll Free (844) 818-1088
(TTY 711) to assist. We will reach out to your provider to explain.

Do I still use my Medicare card if I choose to enroll in an MAPD plan?

No. Put your Medicare card in a safe place in case you need it at a later date. You will use only your
Humana ID Card for medical and prescriptions.

Is there a mail order pharmacy?

Yes, Humana has their own Mail Order Pharmacy. Humana Mail Order Customer Service can be reached
at (800) 379-0092.

Do I need Prior Authorizations for certain Medications?

Some drugs may require a Prior Authorization. Please contact RetireeFirst at (360) 205-1415 or Toll Free
(844) 818-1088 (TTY 711) if you have questions or need assistance with Prior Authorizations as well as
any other requirements such as Step Therapy, Quantity Limit, or Formulary Exceptions.

Am I covered if I travel outside the country?

There is foreign travel emergency coverage on both the MAPD plans as well as the Medicare Supplement
Plan. Please refer to the plan booklets for addition details or contact your Retiree Advocates at (360) 205-
1415 or Toll Free (844) 818-1088 (TTY 711).

What do I do if I lose my Member ID Card?

Please call RetireeFirst at (360) 205-1415 or Toll Free (844) 818-1088 (TTY 711) and we will obtain a
new one on your behalf, mail you a temporary ID card, and call your pharmacy and/or providers if
needed.

Are there any extra benefits included with the Humana MAPD Plan?

Yes! The Humana MAPD plan includes Silver Sneakers. You will receive your Silver Sneakers ID card
about 3-6 weeks after the plan goes live. Please visit www.silversneakers.com, call Silver Sneakers at
866-584-7389 or call RetireeFirst at (360) 205-1415 (TTY 711) to learn more about this benefit and to
view locations.

What does the Humana Vision Benefit Include?

$0 copay exam and $150 per year towards frames/contact lenses.

Your vision benefit is through EyeMed. Please contact RetireeFirst if you need assistance at (360) 205-1415 (TTY 711).

  • See a provider in the EyeMed network and hand them your Humana ID card.
  • You may see a provider outside of the EyeMed network. The provider can bill EyeMed or you can submit a reimbursement claim form directly to EyeMed.

What does the Humana Hearing benefit include?

$0 copay hearing aid exam and $500 towards hearing aids once every 3 years.
Please follow the below steps when utilizing your Hearing Benefit. Please contact Labor First if you need
assistance at (360) 205-1415 (TTY 711).

  • See a licensed Audiologist who is willing and able to bill your medical plan for your exam.
  • Choose your hearing aids in the same facility if they are willing and able to bill your medical plan.
  • You may also take your prescription to a local retailer or order online. You will have to complete a Direct Member Reimbursement Form (DMR) with Humana. You will pay upfront and will be reimbursed directly from Humana.
