As part of the IAFF Health & Wellness Trust’s transition to MagnaCare for billing and eligibility administration effective September 1, 2026, there are important changes to how self-pay premiums will be processed.
This notice only applies to retirees and COBRA participants who make monthly medical and/or dental premium payments directly by ACH or check. If this change impacts you, you should receive a letter in the mail from the Trust Office with instructions and a form.
What You Need to Do – Complete and Return the new ACH Authorization Form no later than August 12, 2026:
- Complete and sign the ACH Authorization Form
- Include a voided check, or a clear image/photo of a check showing your routing number and account number

What is Not Changing
If your retiree premiums are deducted directly from your DRS or PERSI pension payment, no action is required. Your premiums will continue automatically, and you do not need to complete a new authorization.
If you are mailing checks you may continue to do so, but payments received after September 1, 2026 should be sent to:
